SMS Terms & Conditions
These terms govern the text messaging (SMS/MMS) programs operated by each of the practices listed above. Each practice operates its own program for its own patients and customers.
1. Program description
By opting in, you agree to receive text messages from the practice you have a relationship with, including: appointment reminders, confirmations, and rescheduling notices; eyewear and contact lens order status; prescription, refill, and annual exam recall notices; billing and account notifications; and replies from our staff to questions you send us.
2. How you opt in
You may give express written consent to receive text messages by any of the following:
- Providing your mobile number and checking the SMS consent box on our patient intake, registration, or appointment request form, in print or online.
- Giving verbal consent to a staff member in person or by telephone, which we record in your file.
- Sending a text message to the practice number first.
Consent to receive text messages is not a condition of any purchase, service, examination, or treatment. You may receive care and buy products without agreeing to text messages.
3. Message frequency
Message frequency varies and depends on your appointments, orders, and account activity.
4. Cost
Message and data rates may apply. Charges are set by your mobile carrier and are your responsibility. Check your mobile plan for details.
5. How to opt out
You can cancel at any time. Reply STOP to any message from us. You will receive one final message confirming that you have been unsubscribed, and you will not receive further texts from that practice. To rejoin, opt in again using any method in Section 2.

7. Supported carriers and delivery
Delivery is subject to effective transmission by your mobile carrier and is not guaranteed. Carriers are not liable for delayed or undelivered messages. We are not responsible for messages that are delayed or not delivered due to carrier issues, network conditions, or a change to your device or number.
8. Changes to your mobile number
Tell us promptly if your mobile number changes or is deactivated so that messages intended for you are not sent to someone else. You agree to indemnify us against claims arising from your failure to notify us of a number change.

10. Health information in text messages
Standard text messaging is not a secure or encrypted channel. We limit the clinical detail included in text messages. Do not send emergency, urgent, or sensitive medical information by text. In an emergency, call 911 or go to the nearest emergency room.
11. Changes to these terms
We may update these terms. The effective date above shows when they were last revised. Continued participation after an update means you accept the revised terms.
