This Privacy Policy explains how EyeLeader Group and the three practices listed above (“we,” “us,” “our”) collect, use, and protect information you provide through this website and through our patient communications, including text messaging.

- Information you give us: name, mobile and home phone numbers, email address, mailing address, date of birth, insurance information, appointment preferences, and any message you send us through a form, by email, or by text.
- Consent records: the date, time, method, and wording of the consent you gave to receive text messages, retained as proof of opt-in.
- Technical information: IP address, browser type, device type, pages viewed, and date and time of visit, collected automatically in standard server logs.

We retain personal information for as long as needed to provide care and services, and afterward for the periods required by medical recordkeeping, insurance, and tax laws. Text messaging consent records are retained for as long as you remain opted in and for a reasonable period afterward as proof of consent.

- Email: use the unsubscribe link in any non-clinical email.

9. Children
This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this website. Information about minor patients is provided by a parent or legal guardian and handled as part of the patient record.
