This Privacy Policy explains how EyeLeader Group and the three practices listed above (“we,” “us,” “our”) collect, use, and protect information you provide through this website and through our patient communications, including text messaging.

1. Information we collect

Information you give us: name, mobile and home phone numbers, email address, mailing address, date of birth, insurance information, appointment preferences, and any message you send us through a form, by email, or by text.
Consent records: the date, time, method, and wording of the consent you gave to receive text messages, retained as proof of opt-in.
Technical information: IP address, browser type, device type, pages viewed, and date and time of visit, collected automatically in standard server logs.

Health information created or received by our practices in the course of providing care is protected health information under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. It is governed by each practice’s Notice of Privacy Practices, which is provided to you at the practice and available on request. We use administrative, technical, and physical safeguards designed to protect personal information against loss, misuse, and unauthorized access or disclosure. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

We retain personal information for as long as needed to provide care and services, and afterward for the periods required by medical recordkeeping, insurance, and tax laws. Text messaging consent records are retained for as long as you remain opted in and for a reasonable period afterward as proof of consent.

Email: use the unsubscribe link in any non-clinical email.
Phone: ask us to change or stop a type of communication.
Your records: you may request access to, a copy of, or correction of your information by contacting the practice that treats you.

This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this website. Information about minor patients is provided by a parent or legal guardian and handled as part of the patient record.
